Question: What is the cause of the characteristic "ground-glass" appearance of hepatocytes in HBV infection?  
Answer: The 22-nm-diameter HBV surface antigen particles accumulate in the ER. ER expands to accommodate the increased particles. The hepatocytes become enlarged, hydropic, and eosinophilic and display the characteristic features of "ground-glass" cells.
