The Division of Histology and Immunohistochemistry is an integral part of the Anatomic Pathology Branch. Pathology laboratories at University Hospital, University Hospital East, and Memorial Hospital of Union County in Marysville, OH submit a combined average of 762 tissue cassettes daily for processing, embedding, and cutting for slide preparation. A laboratory staff of 1 manager, 13 full-time and 3 part-time histotechnologists, 2 full-time and 1 part-time laboratory assistants, and 1 technician (MLT) meet operational turn-around-time goals by delivery of H&E stained slides and other special-ordered histochemical stained slides to attending pathologists for next day evaluation. The Pathology Department was fortunate to hire a new highly experienced manager, Bonnie Whitaker, for the Histology Laboratory. As transplantation surgery continues to increase so does the demand for same day biopsy results for appropriate patient care. To meet same day processing of STAT transplant biopsy specimens, a microwave tissue processor has been employed to provide H&E stained slides within 3 hours from time of receipt in the surgical pathology grossing laboratory. A “STAT” burn protocol has been established to provide next-day reads on biopsies and debridements from burn patients. This protocol includes PAS/fungus and GMS histochemical stains. The OSU Histology Lab, LLC provides an alternate site for analysis of the surgical biopsy performed in the outreach patient setting. Six professional staff perform grossing, histology, and transcription and achieve H&E slide delivery times that are identical to turn-around-time expectations as compared to the main OSU Histology Laboratory. The Histology Lab, LLC successfully completed an inspection by the College of American Pathologists, and as of May 2007, is accredited by the New York State Health Department. To address the nationwide shortage of quality histotechnologists, we combined efforts with Columbus State Community College and Indiana University College of Medicine to identify and train qualified individuals in the field of histotechnology. On-line course work is available for those applicants with the OSUMC histology laboratory serving as a clinical training site. The first two students have graduated and passed their HT registry examination. Three additional students are enrolled this year. The IHC laboratory, staffed with a manager and three technologists, has a test menu of approximately 140 immunostains, and stains an average of 258 slides per day. Immunostains related to identification of melanoma in sentinel lymph nodes and new immunostain protocols for breast/axillary sentinel lymph nodes have been established. These protocols, ordered along with the H&E stain at the time of grossing, will reduce by one day the time to pathologic diagnosis by the attending pathologist. The Mismatch Repair Protein Panel to determine risk of Lynch Syndrome has been introduced for the evaluation of patients with colon carcinoma. A dual immunostain for melan-A and Mib1 (Ki-67) has been introduced for the evaluation of difficult melanocytic skin lesions. The IHC laboratory has extensively evaluated the available automated immunostainers on the market, in order to replace outdated equipment.
