You will need Acrobat Reader to be able to read the files that are in PDF format. Click on the Acrobat Reader image to download it.

If sending the TAS forms as an electronic attachment, right click on the applicable file below and click "Save Target As" to your desktop. Complete the form from the saved version (must have the full Acrobat program to save the file) on your desktop and resend to TAS as an e-mail attachment.
